In order for us to adequately process our necessary paperwork by the start of school in August, please fill in and submit the forms linked below no later than Fri., June 30. These forms are also included in the parent mailing packet emailed to you.
Required Office Information:
- Communications and Family Information Form (ONE per family)
- Information Form (age 3 – 5)
- We will mail a hard copy for you to fill out, sign, and either mail or email to Cheri by June 30.
- Early Childhood Education Center Parental Agreement Form (age 3 – 5)
- We will mail a hard copy for you to fill out, sign, and either mail or email to Cheri by June 30.
- Up-to-date Immunization Record Form #3231. Please refer to pediatrician’s office for Form #3231 and mail or email a copy to Cheri Menosky before June 30. (all students ages 3 – 5, entering kindergarten and entering sixth grade)
